Adds docs/design/PHP.md, the design record for the PHP back end (step 1 of #64), modelled on JAVASCRIPT-BACKEND.md: module layout and driver, the IR construct by construct, kernel K1 to K8, memory, tasks, the 8-slice table with an acceptance test each, a first slice precise enough to file, and a section listing every PHP-behaviour statement the design leans on (nothing was measured; each is pinned by a named slice).

Also links the record from docs/design/index.md, docs/ROADMAP.md and docs/design/JAVASCRIPT-AND-PHP.md, and amends the tasks-in-PHP text and the "No Fibers" sentence there (decided by the loop: one run queue with stream_select, no Fibers/ext-uv/ext-ev).

Docs only; seed/ untouched.
